mm: factor helpers for memory_failure_dev_pagemap

mm: factor helpers for memory_failure_dev_pagemap

memory_failure_dev_pagemap code is a bit complex before introduce RMAP
feature for fsdax.  So it is needed to factor some helper functions to
simplify these code.
