Remove some unused macros related to the old load/unload catchpoints.

        (SOLIB_UNLOADED_LIBRARY_PATHNAME, SOLIB_CREATE_CATCH_LOAD_HOOK)
        (SOLIB_CREATE_CATCH_UNLOAD_HOOK): Delete. No longer used.
