x86/cpu: Simplify extended APIC ID detection on AMD
Both if-branches are under if (boot_cpu_has(X86_FEATURE_APIC)), unify
them.
Also, simplify the test for bits:
- 17 ("ApicExtBrdCst: APIC extended broadcast enable") and
- 18 ("ApicExtId: APIC extended ID enable.")
in "D18F0x68 Link Transaction Control."
No functionality change.

edfe63ec97ed ("x86/mtrr: Fix Xorg crashes in Qemu sessions")
PAT is now set to disabled state when MTRRs are disabled.
Thus, reactivating the __pa(high_memory) check in
phys_mem_access_prot_allowed().
When CONFIG_DEBUG_VIRTUAL is set, __pa() calls __phys_addr(),
which in turn calls slow_virt_to_phys() for 'high_memory'.
Because 'high_memory' is set to (the max direct mapped virt
addr + 1), it is not a valid virtual address. Hence,
slow_virt_to_phys() returns 0 and hit the BUG_ON. Using
__pa_nodebug() instead of __pa() will fix this BUG_ON.
However, this code block, originally written for Pentiums and
earlier, is no longer adequate since a 32-bit Xen guest has
MTRRs disabled and supports ZONE_HIGHMEM. In this setup,
this code sets UC attribute for accessing RAM in high memory
range.
Delete this code block as it has been unused for a long time.

x86/mce: Avoid using object after free in genpool
When we loop over all queued machine check error records to pass them
to the registered notifiers we use llist_for_each_entry(). But the loop
calls gen_pool_free() for the entry in the body of the loop - and then
the iterator looks at node->next after the free.
Use llist_for_each_entry_safe() instead.

x86/vdso: Remove direct HPET access through the vDSO
Allowing user code to map the HPET is problematic. HPET
implementations are notoriously buggy, and there are probably many
machines on which even MMIO reads from bogus HPET addresses are
problematic.
We have a report that the Dell Precision M2800 with:
ACPI: HPET 0x00000000C8FE6238 000038 (v01 DELL CBX3
01072009 AMI.
00000005)
is either so slow when accessing the HPET or actually hangs in some
regard, causing soft lockups to be reported if users do unexpected
things to the HPET.
The vclock HPET code has also always been a questionable speedup.
Accessing an HPET is exceedingly slow (on the order of several
microseconds), so the added overhead in requiring a syscall to read
the HPET is a small fraction of the total code of accessing it.
To avoid future problems, let's just delete the code entirely.
In the long run, this could actually be a speedup. Waiman Long as a
patch to optimize the case where multiple CPUs contend for the HPET,
but that won't help unless all the accesses are mediated by the
kernel.

x86/cpu: Add Erratum 88 detection on AMD
Erratum 88 affects old AMD K8s, where a SWAPGS fails to cause an input
dependency on GS. Therefore, we need to MFENCE before it.
But that MFENCE is expensive and unnecessary on the remaining x86 CPUs
out there so patch it out on the CPUs which don't require it.

x86/cpu: Probe the behavior of nulling out a segment at boot time
AMD and Intel do different things when writing zero to a segment
selector. Since neither vendor documents the behavior well and it's
easy to test the behavior, try nulling fs to see what happens.

x86/arch_prctl: Fix ARCH_GET_FS and ARCH_GET_GS
ARCH_GET_FS and ARCH_GET_GS attempted to figure out the fsbase and
gsbase respectively from saved thread state. This was wrong: fsbase
and gsbase live in registers while a thread is running, not in
memory.
For reasons I can't fathom, the fsbase and gsbase code were
different. Since neither was correct, I didn't try to figure out
what the point of the difference was.
Change it to simply read the MSRs.
The code for reading the base for a remote thread is also completely
wrong if the target thread uses its own descriptors (which is the case
for all 32-bit threaded programs), but fixing that is a different
story.

lguest, x86/entry/32: Fix handling of guest syscalls using interrupt gates
In
a798f091113e ("x86/entry/32: Change INT80 to be an interrupt gate")
Andy broke lguest. This is because lguest had special code to allow
the 0x80 trap gate go straight into the guest itself; interrupts gates
(without more work, as mentioned in the file's comments) bounce via
the hypervisor.
His change made them go via the hypervisor, but as it's in the range of
normal hardware interrupts, they were not directed through to the guest
at all. Turns out the guest userspace isn't very effective if syscalls
are all noops.
I haven't ripped out all the now-useless trap-direct-to-guest-kernel
code yet, since it will still be needed if someone decides to update
this optimization.

1ae1602de0 "configfs: switch ->default groups to a linked list"
left the NULL gps pointer behind after removing the kcalloc() call which
made it non-NULL. It also left the !gps check in place so make_cluster()
now fails with ENOMEM. Remove the remaining uses of the gps variable to
fix that.

x86/build: Build compressed x86 kernels as PIE
The 32-bit x86 assembler in binutils 2.26 will generate R_386_GOT32X
relocation to get the symbol address in PIC. When the compressed x86
kernel isn't built as PIC, the linker optimizes R_386_GOT32X relocations
to their fixed symbol addresses. However, when the compressed x86
kernel is loaded at a different address, it leads to the following
load failure:
Failed to allocate space for phdrs
during the decompression stage.
If the compressed x86 kernel is relocatable at run-time, it should be
compiled with -fPIE, instead of -fPIC, if possible and should be built as
Position Independent Executable (PIE) so that linker won't optimize
R_386_GOT32X relocation to its fixed symbol address.
Older linkers generate R_386_32 relocations against locally defined
symbols, _bss, _ebss, _got and _egot, in PIE. It isn't wrong, just less
optimal than R_386_RELATIVE. But the x86 kernel fails to properly handle
R_386_32 relocations when relocating the kernel. To generate
R_386_RELATIVE relocations, we mark _bss, _ebss, _got and _egot as
hidden in both 32-bit and 64-bit x86 kernels.
To build a 64-bit compressed x86 kernel as PIE, we need to disable the
relocation overflow check to avoid relocation overflow errors. We do
this with a new linker command-line option, -z noreloc-overflow, which
got added recently:

Add -z noreloc-overflow command-line option to the x86-64 ELF linker to
disable relocation overflow check. This can be used to avoid relocation
overflow check if there will be no dynamic relocation overflow at
run-time.
The 64-bit compressed x86 kernel is built as PIE only if the linker supports
-z noreloc-overflow. So far 64-bit relocatable compressed x86 kernel
boots fine even when it is built as a normal executable.

x86/xen, pat: Remove PAT table init code from Xen
Xen supports PAT without MTRRs for its guests. In order to
enable WC attribute, it was necessary for xen_start_kernel()
to call pat_init_cache_modes() to update PAT table before
starting guest kernel.
Now that the kernel initializes PAT table to the BIOS handoff
state when MTRR is disabled, this Xen-specific PAT init code
is no longer necessary. Delete it from xen_start_kernel().
Also change __init_cache_modes() to a static function since
PAT table should not be tweaked by other modules.

x86/mtrr: Fix PAT init handling when MTRR is disabled
get_mtrr_state() calls pat_init() on BSP even if MTRR is disabled.
This results in calling pat_init() on BSP only since APs do not call
pat_init() when MTRR is disabled. This inconsistency between BSP
and APs leads to undefined behavior.
Make BSP's calling condition to pat_init() consistent with AP's,
mtrr_ap_init() and mtrr_aps_init().

x86/mtrr: Fix Xorg crashes in Qemu sessions
A Xorg failure on qemu32 was reported as a regression [1] caused by
commit
9cd25aac1f44 ("x86/mm/pat: Emulate PAT when it is disabled").
This patch fixes the Xorg crash.
Negative effects of this regression were the following two failures [2]
in Xorg on QEMU with QEMU CPU model "qemu32" (-cpu qemu32), which were
triggered by the fact that its virtual CPU does not support MTRRs.
#1. copy_process() failed in the check in reserve_pfn_range()

A WC map request was tracked as WC in memtype, which set a PTE as
UC (pgprot) per __cachemode2pte_tbl[]. This led to this error in
reserve_pfn_range() called from track_pfn_copy(), which obtained
a pgprot from a PTE. It converts pgprot to page_cache_mode, which
does not necessarily result in the original page_cache_mode since
__cachemode2pte_tbl[] redirects multiple types to UC.
#2. error path in copy_process() then hit WARN_ON_ONCE in
untrack_pfn().

These negative effects are caused by two separate bugs, but they
can be addressed in separate patches. Fixing the pat_init() issue
described below addresses the root cause, and avoids Xorg to hit
these cases.
When the CPU does not support MTRRs, MTRR does not call pat_init(),
which leaves PAT enabled without initializing PAT. This pat_init()
issue is a long-standing issue, but manifested as issue #1 (and then
hit issue #2) with the above-mentioned commit because the memtype
now tracks cache attribute with 'page_cache_mode'.
This pat_init() issue existed before the commit, but we used pgprot
in memtype. Hence, we did not have issue #1 before. But WC request
resulted in WT in effect because WC pgrot is actually WT when PAT
is not initialized. This is not how it was designed to work. When
PAT is set to disable properly, WC is converted to UC. The use of
WT can result in a system crash if the target range does not support
WT. Fortunately, nobody ran into such issue before.
To fix this pat_init() issue, PAT code has been enhanced to provide
pat_disable() interface. Call this interface when MTRRs are disabled.
By setting PAT to disable properly, PAT bypasses the memtype check,
and avoids issue #1.

In preparation for fixing a regression caused by:
9cd25aac1f44 ("x86/mm/pat: Emulate PAT when it is disabled")
... PAT needs to provide an interface that prevents the OS from
initializing the PAT MSR.
PAT MSR initialization must be done on all CPUs using the specific
sequence of operations defined in the Intel SDM. This requires MTRRs
to be enabled since pat_init() is called as part of MTRR init
from mtrr_rendezvous_handler().
Make pat_disable() as the interface that prevents the OS from
initializing the PAT MSR. MTRR will call this interface when it
cannot provide the SDM-defined sequence to initialize PAT.
This also assures that pat_disable() called from pat_bsp_init()
will set the PAT table properly when CPU does not support PAT.

x86/mm/pat: Add support of non-default PAT MSR setting
In preparation for fixing a regression caused by:
9cd25aac1f44 ("x86/mm/pat: Emulate PAT when it is disabled")'
... PAT needs to support a case that PAT MSR is initialized with a
non-default value.
When pat_init() is called and PAT is disabled, it initializes the
PAT table with the BIOS default value. Xen, however, sets PAT MSR
with a non-default value to enable WC. This causes inconsistency
between the PAT table and PAT MSR when PAT is set to disable on Xen.
Change pat_init() to handle the PAT disable cases properly. Add
init_cache_modes() to handle two cases when PAT is set to disable.
1. CPU supports PAT: Set PAT table to be consistent with PAT MSR.
2. CPU does not support PAT: Set PAT table to be consistent with
PWT and PCD bits in a PTE.
Note, __init_cache_modes(), renamed from pat_init_cache_modes(),
will be changed to a static function in a later patch.

x86/mm/pkeys: Add missing Documentation
Stefan Richter noticed that the X86_INTEL_MEMORY_PROTECTION_KEYS option
in arch/x86/Kconfig references Documentation/x86/protection-keys.txt,

x86/topology: Fix AMD core count
It turns out AMD gets x86_max_cores wrong when there are compute
units.
The issue is that Linux assumes:
nr_logical_cpus = nr_cores * nr_siblings
But AMD reports its CU unit as 2 cores, but then sets num_smp_siblings
to 2 as well.
Boris: fixup ras/mce_amd_inj.c too, to compute the Node Base Core
properly, according to the new nomenclature.

This can occur because the SPI controller driver (SPI_PXA2XX) is built as
module and thus not necessarily loaded. While building SPI_PXA2XX into the
kernel would make the problem disappear, it appears prudent to ensure that

f2fs/crypto: fix xts_tweak initialization
Commit
0b81d07790726 ("fs crypto: move per-file encryption from f2fs
tree to fs/crypto") moved the f2fs crypto files to fs/crypto/ and
renamed the symbol prefixes from "f2fs_" to "fscrypt_" (and from "F2FS_"
to just "FS" for preprocessor symbols).
Because of the symbol renaming, it's a bit hard to see it as a file
move: use
git show -M30
0b81d07790726
to lower the rename detection to just 30% similarity and make git show
the files as renamed (the header file won't be shown as a rename even
then - since all it contains is symbol definitions, it looks almost
completely different).
Even with the renames showing as renames, the diffs are not all that
easy to read, since so much is just the renames. But Eric Biggers
noticed that it's not just all renames: the initialization of the
xts_tweak had been broken too, using the inode number rather than the
page offset.
That's not right - it makes the xfs_tweak the same for all pages of each
inode. It _might_ make sense to make the xfs_tweak contain both the
offset _and_ the inode number, but not just the inode number.

NTB: Remove _addr functions from ntb_hw_amd
Kernel zero day testing warned about address space confusion. A virtual
iomem address was used where a physical address is expected. The
offending functions implement an optional part of the api, so they are
removed. They can be added later, after testing.

* switch orangefs_remount() to taking ORANGEFS_SB(sb) instead of sb
* remove from the list _before_ orangefs_unmount() - request_mutex
in the latter will make sure that nothing observed in the loop in
ORANGEFS_DEV_REMOUNT_ALL handling will get freed until the end
of loop
* on removal, keep the forward pointer and zero the back one. That
way we can drop and regain the spinlock in the loop body (again,
ORANGEFS_DEV_REMOUNT_ALL one) and still be able to get to the
rest of the list.

when get_init_ra_size() gets called from ondemand_readahead().
The net effect is that the initial readahead size is arch dependent for
requested read lengths of zero: for example, since
1UL << (sizeof(unsigned long) * 8)
evaluates to 1 on x86 while its result is 0 on ARMv7, the initial readahead
size becomes 4 on the former and 0 on the latter.
What's more, whether or not the file access timestamp is updated for zero
length reads is decided differently for the two cases of IOCB_DIRECT
being set or cleared: in the first case, generic_file_read_iter()
explicitly skips updating that timestamp while in the latter case, it is
always updated through the call to do_generic_file_read().
According to POSIX, zero length reads "do not modify the last data access
timestamp" and thus, the IOCB_DIRECT behaviour is POSIXly correct.
Let generic_file_read_iter() unconditionally check the requested read
length at its entry and return immediately with success if it is zero.

mm, kasan: stackdepot implementation. Enable stackdepot for SLAB
Implement the stack depot and provide CONFIG_STACKDEPOT. Stack depot
will allow KASAN store allocation/deallocation stack traces for memory
chunks. The stack traces are stored in a hash table and referenced by
handles which reside in the kasan_alloc_meta and kasan_free_meta
structures in the allocated memory chunks.
IRQ stack traces are cut below the IRQ entry point to avoid unnecessary
duplication.
Right now stackdepot support is only enabled in SLAB allocator. Once
KASAN features in SLAB are on par with those in SLUB we can switch SLUB
to stackdepot as well, thus removing the dependency on SLUB stack
bookkeeping, which wastes a lot of memory.
This patch is based on the "mm: kasan: stack depots" patch originally
prepared by Dmitry Chernenkov.
Joonsoo has said that he plans to reuse the stackdepot code for the
mm/page_owner.c debugging facility.

arch, ftrace: for KASAN put hard/soft IRQ entries into separate sections
KASAN needs to know whether the allocation happens in an IRQ handler.
This lets us strip everything below the IRQ entry point to reduce the
number of unique stack traces needed to be stored.
Move the definition of __irq_entry to <linux/interrupt.h> so that the
users don't need to pull in <linux/ftrace.h>. Also introduce the
__softirq_entry macro which is similar to __irq_entry, but puts the
corresponding functions to the .softirqentry.text section.

kasan: modify kmalloc_large_oob_right(), add kmalloc_pagealloc_oob_right()
This patchset implements SLAB support for KASAN
Unlike SLUB, SLAB doesn't store allocation/deallocation stacks for heap
objects, therefore we reimplement this feature in mm/kasan/stackdepot.c.
The intention is to ultimately switch SLUB to use this implementation as
well, which will save a lot of memory (right now SLUB bloats each object
by 256 bytes to store the allocation/deallocation stacks).
Also neither SLUB nor SLAB delay the reuse of freed memory chunks, which
is necessary for better detection of use-after-free errors. We
introduce memory quarantine (mm/kasan/quarantine.c), which allows
delayed reuse of deallocated memory.
This patch (of 7):

the test only checks the page allocator functionality. Also reimplement
kmalloc_large_oob_right() so that the test allocates a large enough
chunk of memory that still does not trigger the page allocator fallback.

mm/page_alloc: prevent merging between isolated and other pageblocks
Hanjun Guo has reported that a CMA stress test causes broken accounting of
CMA and free pages:

Laura Abbott has confirmed the issue and suspected the freepage accounting
rewrite around 3.18/4.0 by Joonsoo Kim. Joonsoo had a theory that this is
caused by unexpected merging between MIGRATE_ISOLATE and MIGRATE_CMA
pageblocks:
> CMA isolates MAX_ORDER aligned blocks, but, during the process,

> pageblock_order is 9, two pageblocks make up MAX_ORDER

This was supposed to be prevented by CMA operating on MAX_ORDER blocks,
but since it doesn't hold the zone->lock between pageblocks, a race
window does exist.
It's also likely that unexpected merging can occur between
MIGRATE_ISOLATE and non-CMA pageblocks. This should be prevented in
__free_one_page() since commit
3c605096d315 ("mm/page_alloc: restrict
max order of merging on isolated pageblock"). However, we only check
the migratetype of the pageblock where buddy merging has been initiated,
not the migratetype of the buddy pageblock (or group of pageblocks)
which can be MIGRATE_ISOLATE.
Joonsoo has suggested checking for buddy migratetype as part of
page_is_buddy(), but that would add extra checks in allocator hotpath
and bloat-o-meter has shown significant code bloat (the function is
inline).
This patch reduces the bloat at some expense of more complicated code.
The buddy-merging while-loop in __free_one_page() is initially bounded
to pageblock_border and without any migratetype checks. The checks are
placed outside, bumping the max_order if merging is allowed, and
returning to the while-loop with a statement which can't be possibly
considered harmful.
This fixes the accounting bug and also removes the arguably weird state
in the original commit
3c605096d315 where buddies could be left
unmerged.

drivers/memstick/host/r592.c: avoid gcc-6 warning
The r592 driver relies on behavior of the DMA mapping API that is
normally observed but not guaranteed by the API. Instead it uses a
runtime check to fail transfers if the API ever behaves
When CONFIG_NEED_SG_DMA_LENGTH is not set, one of the checks turns into a
comparison of a variable with itself, which gcc-6.0 now warns about:

ocfs2: extend enough credits for freeing one truncate record while replaying truncate records
Now function ocfs2_replay_truncate_records() first modifies tl_used,
then calls ocfs2_extend_trans() to extend transactions for gd and alloc
inode used for freeing clusters. jbd2_journal_restart() may be called
and it may happen that tl_used in truncate log is decreased but the
clusters are not freed, which means these clusters are lost. So we
should avoid extending transactions in these two operations.

ocfs2: extend transaction for ocfs2_remove_rightmost_path() and ocfs2_update_edge_lengths() before to avoid inconsistency between inode and et
I found that jbd2_journal_restart() is called in some places without
keeping things consistently before. However, jbd2_journal_restart() may
commit the handle's transaction and restart another one. If the first
transaction is committed successfully while another not, it may cause
filesystem inconsistency or read only. This is an effort to fix this
kind of problems.

jbd2_journal_restart() may be called and it may happened that the buffers
dirtied in ocfs2_truncate_rec() are committed while buffers dirtied in
ocfs2_et_update_clusters() are not, the total clusters on extent tree and
i_clusters in ocfs2_dinode is inconsistency. So the clusters got from
ocfs2_dinode is incorrect, and it also cause read-only problem when call
ocfs2_commit_truncate() with the error message: "Inode %llu has empty
extent block at %llu".
We should extend enough credits for function ocfs2_remove_rightmost_path
and ocfs2_update_edge_lengths to avoid this inconsistency.

ocfs2/dlm: move lock to the tail of grant queue while doing in-place convert
We have found a bug when two nodes doing umount one after another.
1) Node 1 migrate a lockres that has 3 locks in grant queue such as
N2(PR)<->N3(NL)<->N4(PR) to N2. After migration, lvb of the lock
N3(NL) and N4(PR) are empty on node 2 because migration target do not
copy lvb to these two lock.
2) Node 3 want to convert to PR, it can be granted in
__dlmconvert_master(), and the order of these locks is unchanged. The
lvb of the lock N3(PR) on node 2 is copyed from lockres in function
dlm_update_lvb() while the lvb of lock N4(PR) is still empty.
3) Node 2 want to leave domain, it will migrate this lockres to node 3.
Then node 2 will trigger the BUG in dlm_prepare_lvb_for_migration()
when adding the lock N4(PR) to mres with the following message because
the lvb of mres is already copied from lock N3(PR), but the lvb of lock
N4(PR) is empty.

ocfs2: solve a problem of crossing the boundary in updating backups
In update_backups() there exists a problem of crossing the boundary as
follows:
we assume that lun will be resized to 1TB(cluster_size is 32kb), it will
include 0~
33554431 cluster, in update_backups func, it will backup super
block in location of 1TB which is the 33554432th cluster, so the
phenomenon of crossing the boundary happens.
